1.       Orders need to be placed for all age groups that have already formed teams by May 10^th (U9 - U14) on the order form posted above

2.       Dayak’s Den deadline with Adidas to change/add orders is May 15^th so it is imperative that we have this information

3.       50 % Uniform Deposit is due June 15^th.  We will not start on the teams uniforms until we have the deposit

4.       Deposit is payable by Team check only.  Please do not submit 15 different parent checks, this is not acceptable

5.       Remaining Balance is Due at time of delivery/pick up.  Teams will not be able to pick up without payment in full
